Professional Agreement
Welcome to our practice. We look forward to working together. There are a few guidelines that are helpful for clients to familiarize themselves with in order for our relationship to work. If you have any questions, please let us know.
Services: Services are provided either in person (at our Palo Alto office) or via tele-sessions. The client acknowledges that coaching is a comprehensive process that may address multiple areas of life, including work, finances, health, relationships, education, and recreation. The client understands that coaching is not a form of mental health treatment and does not replace clinical or medical care if needed. Coaching does not diagnose, prevent, cure, or treat any mental health condition or medical illness.
Payments: Payment is due on the Attune-iN payment platform 48 hours before the appointment. Fee arrangements will be discussed ahead of time with the client.
Cancellation: A 48-hour notice is required for rescheduling or canceling an appointment. The full session fee will be charged for missed sessions without 48-hour notification.
Confidentiality: All information disclosed during our sessions is confidential. Information may not be revealed to anyone without your written permission. However, please be aware that the coach-client relationship is not considered a legally confidential relationship (like the medical and legal professions), and thus communications are not subject to the protection of any legally recognized privilege.
Contacting Me: We are often not immediately available by telephone or email. While we usually work Monday through Friday between 8 a.m. and 6 p.m., we will probably not answer the phone or answer emails when we are supporting other clients. We monitor our voicemail and check our emails frequently and will make every effort to return your call/email as soon as possible, and typically within 24 business hours.
Emergency Plan: In emergencies, you can attempt to reach your coach directly and leave an appropriate message.
You express understanding that your coach may not be able to provide immediate emergency services.
If you are experiencing an emergency, please call the emergency medical phone number in your county or proceed to the nearest emergency room to seek help. In addition, if you are having a life-threatening emergency or any suicidal thoughts or are making plans to harm yourself, please call the local Crisis Hotline in addition to contacting the police or proceeding to the nearest emergency room.
The Use of Social Media: I do not accept friend requests from current or former clients on any social networking site. Doing so blurs the boundaries of our professional relationship and possibly compromises both privacy and confidentiality.
Tele-Session: Though tele-sessions are convenient, there are still some issues during tele-sessions, such as technical failures, internet issues, etc. Also, although we utilize a HIPAA-compliant, encrypted video service as well as phone calls, it is possible that the transmission could be accessed by unauthorized persons. This possibility exists with all electronic communication.
